The campaign kicks off with Reverberations, plunging the team into the Chicago drug trade where a new substance threatens to tear the fabric of reality. The nightmare rapidly escalates in Viscid and Extremophilia, where rogue genetics and unnatural contagions demand extreme containment measures. Agents will be forced to make impossible choices, balancing the preservation of human life against the absolute necessity of keeping the cosmic truth hidden.

As the stakes rise, the team must confront haunted suburbia in Music from a Darkened Room, untangle a botched overseas mission through playable flashbacks in The Star Chamber, and face an apocalyptic breach of physics in the grand finale, Observer Effect. As the agents dig deeper through the city's underbelly—navigating shady nightclubs, South Asian fronts, and hostile street corners—they will cross paths with dangerous dealers, secretive associations, and the tragic victims of the substance. The climax centers around a cheap hotel room belonging to a key figure named Spider J. Here, the true, destructive nature of the unnatural threat is unleashed in a scene of inexplicable carnage, forcing the agents to confront the impossible.

The current team finds themselves in a grim interrogation room inside a Chicago brick factory, tasked with judging the actions of a previous squad whose mission in Myanmar ended in utter disaster. Rather than simply reading a report, the players will experience the catastrophic events through interactive flashbacks, seamlessly shifting between the sterile, high-security present and the humid, chaotic past. In the flashbacks, the ill-fated team lands on the muddy banks of the Salween River and ventures into an impoverished agricultural village in Shan State. Beneath the dusty surface and quiet rice paddies lies a dark reality: the village is heavily influenced by the Chauchua (Tcho-Tcho), harboring grotesque secrets, hidden brothels, and hostile locals. The investigation inevitably spirals out of control, leading the agents into a deadly ambush within a dense jungle ravine choked by a supernatural red fog. The climax forces the squad beneath a massive, ominous hill into a nightmarish subterranean temple. Here, amidst unholy Aklo inscriptions and cannibalistic threats, they must confront a grotesque idol. The mission culminates in absolute chaos on the hill's esplanade, with desperate hand-to-hand combat in the dirt and the terrifying roar of approaching fighter jets threatening to wipe the entire site off the map.

Time distortions, erratic equipment behavior, and the frayed nerves of the isolated research staff hint at a looming catastrophe. The situation rapidly deteriorates, leading to a disastrous localized reality breach. The climax forces the agents into a desperate fight for survival and containment. Space itself twists within the facility, giving birth to a bubbling black mass that defies known physics. Bathed in an unnatural blue-white light, the team must confront this destructive extrusion and find a way to sever its connection to our world before it consumes everything.
